Provision of Your Care
While you are staying at Concordia Place, we encourage you to participate in making decisions about your health care.
An interdisciplinary team is involved in the provision of your care.
These professionals include:
- Physicians who determine your needs, prescribe medications and tests that may be required.
- Registered Nurses, Licensed Practical Nurses and Health Care Aides who provide quality care to you 24 hours a day.
- The Admissions Coordinator who arranged your admission will be available to you and your family to provide support and information.
- Clinical Managers and Clinical Resource Nurses who coordinate care are available to answer any questions you may have.
- Recreation staff who offer a wide range of recreation and social programs.
- A Chaplain who provides spiritual and emotional support for you and your family.
- A Clinical Dietitian and Food Services Clerk who will visit you to discuss your eating routines and nutrition/diet preferences.
- Occupational Therapist (OT) who can assist you in choosing equipment such as wheelchairs and cushions.
- A Rehabilitation Assistant who will provide individual exercise programs following at OT or Community Therapy Services (CTS) physiotherapy assessment.
- Physicians who determine your needs, prescribe medications and tests that may be required.
It is important for you and your family to have the opportunity for decision-making regarding certain aspects of your care.
We request that you consider the following:
- In order to promote optimal health, Concordia Place practices an annual influenza vaccination campaign.
- You will be asked to provide consent prior to receiving this vaccine according to the established practices for immunizations in long term care facilities.
- In the event that your health condition should change, we need to know what your wishes are for medical intervention.
- Please provide us with a copy of your “Advanced Health Care Directive, or living will” if you have one. The Health Care Team will discuss the Advanced Health Care Plan with you and a level of care will be determined. Please feel free to contact your Physician, Chaplain, Clinical Manager, or Admissions Coordinator if you have any questions.
- Concordia Place does not administer CPR (cardiopulmonary resuscitation). Residents wishing CPR will be transferred by ambulance to hospital.
You will meet a variety of support staff members throughout your stay.
These people include:
- Finance Services staff who receive your payments for the daily personal care home charges and additional expenses.
- Maintenance staff who are available to hang pictures, perform minor repairs and safety check electrical equipment.
- Housekeeping staff who will provide cleaning services.
- Laundry Aides who provide personal laundry service.
- Volunteers who help enrich your stay by assisting with recreation programs, social outings and spending time with you, doing what you enjoy.
- Students – Concordia Place promotes quality care and services through well educated staff. As part of our commitment to education, we accept a variety of students in health related programs for practice experiences. All of these experiences are supervised.
