Care Conferences

Shortly following your admission and annually thereafter, the Admissions Coordinator, or Clinical Resource Nurse/Clinical Manager together with other members of the Care Team will invite you and your significant others to a Care Conference. This will allow us to get to know you and your likes, dislikes and any issues that require attention.

Care Conferences provide the opportunity for all of us to share information, ask questions and better address your wishes to ensure your stay with us is as positive an experience as possible. You will be provided with a copy of your care plan, current medication list and a summary of the meeting.
